Actually this is exactly what causes it (assuming they don't keep changing their output settings for episodes, which would be odd). You may want to look up some of the basics of video encoding/transcoding, if you want to learn more about why.

Probably not, but, it was rather infuriating how much they removed and changed to make it 12+

Can't say. Not many, but they wanted it widely available, which it really isn't. I don't think they understand the western markets.
