It's not an anime, it's a vn, a direct sequel to tomoyo's route in clannad (http://vndb.org/v12)

if my memory serves me correctly, in that vn tomoya got into an accident which results in him having a coma (for how long I can't remember) and when he's back conscious he lost all the memory about him & tomoyo, 3 years have passed with tomoyo taking care of tomoya (though tomoya doesn't remember about her), he had made a lot of memories with her and he presumably died in the sunset, leaving tomoyo FOREVER ALONE ;_;
she had to wait for tomoya to wake up only to find out he lost all his memories about her and him leaving her 3 years later...

So yeah, it's all true =w=
