ikemen/twig/beautiful boy/attractive guys is is the worst hetero hentai genre, beautiful/cute girl x handsome guys is just so boring and makes dick limp

most hentai enjoyers like masculine or ugly men because of the gap difference (cute x ugly, small x big, human x nonhuman etc) and for faceless,faceless is the best choice for selfinsert

(you will understand this if you have been on this media for a long time and explored many genres)

Personally, my tierlist is:
  1. Top tier: textured fat champs (taka84, blendy, MiPoZ)
  2. High tier: not shown/first person (sugardust, flim13, ddd_suke)
  3. Mid tier: anonymous with a sack on head (dopurinu, maloxx, Kinhage), shota, MC from the source material
  4. Low tier: insects (o-dio), beasts/monsters (JygreAnimation), unnaturally uglified men (doujinvideo)

Im not into some pretty face character as main lead,maybe that ok for simple vanilla romance lovey dovey.
i think the reason lot of ppl love muscular and ugly character often used in H mmd is because the contrast when used with beautiful model (yeah fetish).

if i see that aether as main character there a chance i think that im watching a yuri themed rather than normal one lol.and i think the more ppl dive into Hentai the more their fetish change into more advance lvl and i think pretty boy model as ML is at lvl 1.

also i dont think it more realism when ugly model used compare to good looking one.

The sin sacks, 罪袋, have been around the japanese side since absolutely primordial times. MMD should be from 2008? And the aforementioned guys were around earlier in some touhou videos.. it's not really a question of why, they're just legendary status and easy to handle since the model is... Well, that.
Also, I personally find them goofy but serious, which works damn well in about 99% of the possible scenarios and animations one may come up with.

The ugly bastards are simple: Japanese side of fetishes. It's kind of that simple.
The big ugly creepy dude that suddenly can do whatever with whomever. Need I say more?

I remember being incredibly confused the first time I saw a sin sack character but over time, the sin sacks have become the equivalent of visual 'white noise' for me in that somehow my brain completely tunes them out of any scene, which is probably the point.

..... I had to look up a sin sack to double-check the 'too muscular' part because I straight-up had never noticed, or maybe I just forgot & blocked it from my memory. To your overall point, this is a very effective & functional character for scenes and I prefer that to the H-anime UB types.

MipoZ is exactly the creator i had in mind when i said i have love-hate relationship with videos that have top notch animations and sex scenarios but questionable male cast XD but those kissing while pumping continuous creampies is just too good that it overcompensate the flaws

speaking of no.1, what about Meet? i think the sunglasses fat man character is still palatable when they're not deliberately designed to be ugly & dirty
