Usually it would relate to a character acting out of character. Or maybe a plot hole too big to ignore.
Also the way they speak too. If the characters are in a medieval settings and they use modern slangs then it would be too weird

I guess if the MC is way too enabling to a LI's bad behavior, it breaks the immersion. If he's constantly mistreating and talking down to the MC, I can't stand it. This is especially true if he's romantically involved with her. While I get some people enjoy the trash fire, it's just often badly written and boring to me. It's more interesting if the MC shows resistance in those sort of scenarios. Granted, there are exceptions as always. I enjoy plenty of Rejet games like anyone else.

Or, in scenes where a group gathers and the dialogue should bounce around dynamically, it was always just two people talking while everyone else stood in the background like cardboard cutouts. It was so jarring, and I ended up dropping the game because I was so disappointed with the writer's skill.

Sometimes translations chooses words that do not fit with the set time or the personality of the character. Like seeing a character from a period piece say "yeah" or modern slang, it just takes me out immediately.

That too!! Im always thinking about their surroundings but I've noticed is really common that the characters get lost in their dialogue without minding the space they're in.
I remember this one scene in ozmafia where mc and li start getting affectionate almost in a sexual way while another li was supposed to be in the same room but he never gets addressed, i just felt uncomfortable and couldn't concentrate in whatever was happening
