Searching for source of this image

Hello All,

I have been trying to determine what the title or creator of the source of the below image is for multiple years. I can only find one clip on NuVid that doesn't give any real credit or properly name the work. As the people here seem to be knowledgeable and I have exhausted my other avenues of discovery I thought I would give this forum a shot at it.
