If body shaming counts, Ren Ichinose from Otometeki Koi Kakumei★Love Revo!! definitely qualifies.
This man's mouth is brutal, and his route is basically the hardest one lol. You literally have to lose weight, maintain your grades, buy him gifts, and endure his constant insults.

Most of the main guys (+ other servants and one mom) in Hanayaka Nari Waga Ichizoku call you a variety of names, treat you like crap, and are just generally mean/flippant af in the beginning. But it makes sense in context as they are the master while you are just a servant. They are all absolutely great too.

This is funny because I was actually going to recommend another bodyshaming game, Burn Your Fat With Me! FG.
It was released in 2013 by Creative Freaks (appropriate name) and is still up on the Play Store. It's a part-visual novel, part-fitness app, where you do real-life exercises like sit-ups or squats to progress the story and your affection points with each character.
The main guy shown on the cover is super mean and literally calls you "fatty" and "tubby," among other insults. You could even set daily alarms where he calls you fatty and tells you to work out (I can't imagine being in public with that going off lmao). If i recall correctly there was even an English dub??
FG stands for For Girls, they initially released Burn Your Fat With Me aimed at guys with female love interests. What's crazy is that the love interests in that game aren't mean to the player at all!!!! They are supportive, sweet and kind to the player. The love interests are only mean in the version for girls!
Maybe I shouldn't have been playing this at 14 years old but if you want mean guys, this game has it lol.
