I just tried it again and at my side and it works.

I'm running windows 7
and have all my settings set to japanese see: http://www.anime-sharing.com/forum/hentai-games-38/applocale-unicode-setting-tutorial-40/

then it's easy:

-Mount the .mdf file with PowerISO or a simular program
-setup starts if autostart is enabled. or you just have to double click on the mounted cd in the Computer folder
-when setup is finished you're ready to play
