Honestly, every time I listen to this drama CD, I end up curling up because the VA is literally screaming and crying right into my ears. He acts so well that no matter how many times I listen to it, I still can't get used to it. And this is pretty much the only volume I really like. The other volumes are... umm... maybe they're just not enough for me? Or maybe it's because I listened to Gold and Silver Spider's Thread first, so everything else feels kinda weak compared to it.
Also, most CDs from BoneCage are either bloody horror or just straight up perverted as hell. Most of their CDs are made for girls who are at least a little bit into masochistic stuff though so if that's not your thing, you might wanna be careful
Are you only into angsty otome CDs or do you also want to try some darker stuff? Like rape, drugs, kidnapping, being locked up, etc.?
If you're okay with darker themes, there's Bungou tachi no Cafune. It's been a few years since I last listened to it so I don't really remember if anyone screams or cries, but someone definitely dies. Either the MC dies or the guy dies.
My favorite is Vol. 2 because this guy is fucking perverted lol. Oh, but this volume does have incest in it! I don't remember if the other volumes do or not, but I remember this one has both incest AND age gap, so if you're not okay with that, you can just skip it.

If you want something that's both angsty and has a plot that makes you actually use your brain, there's Ability120%. Umm, how should I explain it... I like this one because it has some gore. The SFX and the sounds of men crying and suffering are so good I was literally sweating while listening to it lmao
Most CDs from Dusk usually have a little bit of gore too, although it's mostly just SFX.

if I had to pick my favorite from everything I mentioned, it would definitely be The Melancholy of Sleeping Beauty and Her Protectors ~ Gold and Silver Spider's Thread ~.
I've listened to plenty of fucked up drama CDs, but there are very few that make me curl up while listening to them. This one really does that to me every single time.
And I think it fits what you're looking for pretty well too, even though nobody actually dies.
I feel like you've probably already listened to some of the CDs above since they're pretty popular. But if you know any other CDs where men are crying and suffering please please recommend them to me too

I don't really know how to say this without spoiling things, but when they force themselves on the MC, the characters aren't enjoying it at all. They usually do it when they're so desperate, suffering, and so full of yearning that they completely lose control of themselves. In some of the CDs, the characters actually feel really guilty and regret doing it. The first CD I recommended is a good example (seriously the way he sounded so hopeless made me feel hopeless too

It's one of my favorites too. Especially the Golden and Silver Spider's Thread volume, which is the one you mentioned. I didn't enjoy the other volumes in the series as much either, but I really loved this one. Definitely seconding this recommendation. 
I'd recommend Bloody Endings Series Vol. 2, Yuki no Joo Hen. It's definitely quite tragic and emotionally heavy, and the ending stayed on my mind for a while. I don't want to spoil too much, but if you're looking for something with a lot of angst and an emotionally devastating ending, I'd definitely recommend it.
