For me, its the story. Even if I like a voice actor a lot, I can't for the life of me remember names. But if I like someone's voice, it makes me interested to check the rest of their works. Otherwise its always the story.
I suppose it depends on the artist. All artists want to evolve so in early stages they experiment more with their styles. Some even keep evolving all the time to go with the current trends. If an artist likes an old style painting, they'll use it.

I would say it depends on the game. There are definitely some that if enough time has passed so the story won't be fresh on my mind, I'd play it again as long as I enjoyed it.
