Otome games about villains/ isekai. I read a lot of shojo manga about villains but seems can't find otome about it except Thorn for the Villian.Any recommendations?

I'm not sure if this would fit your taste but you might like "Cupid". It's an English Visual Novel game (I consider it as an Otome because there is romance and you play as a woman there) that is free and available to play on Windows or Linux. It doesn't have the Isekai part you're looking for but it fits on the 'Villain' genre you're looking for. As for who's the villain, you'll have to think well if it was the male lead (Guilleme) or the character you play on (Rosa) through the scenes that were about to unfold on the game.
That was one of the top-tier Visual Novels I played back then that I still couldn't forget now. The scripting and characters all fit in one perfect puzzle as well as the gameplay that's quite unique. I'll have you warned though, it isn't for the faint of heart, not in a scary horror way but it has themes that make you question your own sanity.
The game was set on european times where witches are simply looked down upon. You know like.. when you're extremely beautiful and men decides "that's not possible, you must be a witch!"-kind of era.
In the game, you kind of play as Rosa, an innocent woman (at first) who's the best friend of the female lead named Catherine. But your choices were in the perspective of her dead mother. Kind of like you're the voice of her reason (although her mother's... quite scary
). So the otome part wasn't exactly about which guy you would like to date in the game but it's more like.. you're trying to deduce the male lead's motives and true feelings named Guilleme (I'm not sure if this would fit in the Otome part but... As far as I know.. Otome is a tag that indicates 'For girls' so I think it still fits what you ask for). In the game, Guilleme claims to love Catherine but as Catherine's best friend, you'll have to find out if Guilleme's a healthy match or nahh...
I won't spoil you about the more things you'll find in the game that can be shocking. But I hope this has peaked your interest. I'm sorry I kind of yapped, but I just really liked this game and hoped you'd like it too
