I tested the DL version of the original game with this extension package:
you can ignore the extend.exe and just move extend.pac to the installation folder; if you see "濃いめ" on the left bottom corner when running the game through the original "okami.exe", then it works (guaranteed by the readme).
I guess what the extend.exe tries to do is just copying the extend.pac file to the installation folder (for those who never manually organise files on their computer?).

I guess you need to double-click?
Did you mean that the game does not run, or you cannot observe any difference from the unpatched version?

BTW, most of エレクトリップ's games are written by softpal's engine, so you do not need to worry about locale setting (probably. I have not done extracting for a long time.).

Did you just move the okami.exe to this extension package's folder?
The extension package is just a patch, it requires all (most) files from the original game.
By missing any one of .pac files, the game at least still generate a black window. I guess you either miss Pal.dll in the "dll" sub-folder, or miss everything beside okami.exe.

BTW, the readme suggests to turn off UAC on Win7/8 OS. It might need the permission to create savedata, but I am not sure whether or not there is an error message if you do not run okami.exe as admin.

I was talking about the original game, not the expansion. And I tried to run it as a admistrator bu it didnt change anything. Feeling kinda helpless^^.
