Thanks a lot for your answers. Actually the survey was made on the fly and now that I'm reviewing it, it has some mistakes in it. Some info will become not reliable for me because of it, but it will still be interesting for any NTR fan. People is asking why I left out some things out, and the reason is this:
I'm going to make a NTR game (yes, another want-to-be-developer appears lol). When I was writing my story, I ran into some problems that made me question my initial intend. At first I wanted to do something simple, because it's my first VN, but as the story developed, new branches routes naturally appeared. I begin to wonder what would happen if I explore those branches and the end result was a somewhat complex decision tree. Since that kind of complexity defies my initial intend (make something simple), I wanted to go back and eliminate some routes, but some of the decisions seemed just so natural to me, that it was hard to eliminate them (I mean I think it's such a waste forcing a given route into the player, instead of making the player decides what he wants to do on that point). That kind of decision tree also allows to explore various different kinds of NTR situations and endings. But if you read my past comments, I've been criticizing the last ANIM games, because they try to focus on too many things, so I realized maybe I was making the same mistake. This led me to my actual predicament:
What do I sacrifice?
Focus (on favor of variety and fluency).
Fluency and variety (on favor of focus).
So I thought into making that survey, to have a clearer idea of what the players actually wants from a NTR work in general. As the survey is focused on the points I'm having doubts I let out a lot of NTR elements, mostly because I was sure of them: For instance the hidden-sex, phone call, locked camera angle, photos, videos, discovery, confession, antagonist taunting, shadow sex, sex sounds/moans/bed-creaking... etc... Those elements are something I have present all the time, I only have to decide which to use in a given story and when. That's also the reason I didn't include more netorase questions in the survey, because when I laid out the story, the netorase route flow was so natural, I didn't have any problems at all with it, the other routes were the ones that gave me problems (I don't consider this an spoiler because at this point nothing is set in stone yet).
So in short this is a survey mostly for my own interests, but also to gather some information I was curious about (like the kind of netori guy physical appearance people prefers). I hadn't made it with the purpose of being an extensive source of gathering NTR preferences. However, since people seemed to like it I think it's a good idea to make a new more extensive one with all the options we can have. So if no one else wants to do it I'll do a new one with more options and corrections (also because there are some questions and answers that are not formulated right and give unreliable data).
So you can give me your thoughts of what else should be added to the survey. When it's done, I'll publish a draft here for revision and if there is no more problems, I'll upload it in a new thread for you people to fill.
Definitely share the results, I also think you should make a topic for this though. Its just gonna get buried in this thread otherwise, with its own thread as you post result people can discuss it too.
Personal note I don't think your questions allowed for netorase answers at all as forms of NTR outside of "do u like netorase or netorase ending" lol.
I already answer this above. When I made the story I initially thought of two main routes: netorare-netorase to make the player decides which he/she prefers. Then as I wrote my story I realized there was a point earlier on the story that will need a decision from the player (to not feel forced). That's when all the problems and complexity begun. I may discard a lot of the concept, but I'll keep the characters, since I already have them well thought up (maybe put them in another situation, or maybe in the same situation if I can manage to fix the problems).
Interesting initial results. I think it might be worth making a new thread with this survey so it doesn't get lost. Or maybe a wider census of us degenerates to include options such as "write down favorite title". Someone left a comment about hidden sex, so favorite netorare situation seems like a good one that was missed (hidden sex, filmed footage, confession from heroine, taunt from antagonist etc). I think people should give opinions on good questions.
I already answered the question in lines above: The reason I didn't put them was because I'm clear on those elements and I have a very good idea of when to use them. Given the nature of the story I might have to let some out, but I understand their importance, that's the key point here. I'd rather go for a fluent storytelling, rather than force things just to include some elements in the story. I think fluency is an element that the best developers can put in their stories (like Rascou or Venus, you can really see how their stories just flow naturally).

Favourite is Reverse NTR:
This might have to be included in the 2.0 version of the survey. Any more suggestions about it are welcome, since I'm not really familiar with that genre.
Prefer option to avoid the NTR since its gives it a more impact:
That's an interesting view. However how would you prefer it?
a. The heroine already cheated but the MC forgives her and they lived happily ever after (although with some scars) ever after. (True happy ending... sort of).
b. The MC avoids to be NTR altogether and the heroine doesn't ever cheat on him. I think this should be pulled out earlier in the story (and earlier ending before the middle of the story).
NTR mystery is my absolute favorite, especially when a game can be played through without seeing any H-scenes. Having only verbal and visual clues to imply the heroine has been cheating is great:
It's really powerful when is well done. Also what do you think is best?
a. When the heroine begin to give hints from the start (of the cheating).
b. When the player (not the MC) realize she is already cheating for a while (the MC begins to discover suspicious stuff when is already too late).
Dont ming fat/ ugly so long as they aren't sickening to look at (often they go to far)
I agree. fat/ugly is one of my favorite types... Not really ugly though, just not really attractive (by most people standart). But I've seeing (especially lately) that sometimes artists go overboard and I don't like it. People like the ones from

Sawamura Ai-san (26) no Kienai Genei or
Saimin Seishidou are ok in my book (and in fact they are among my favorites).
Question 2: It Could had options of family members like mother, sister and maybe even daughter.
Since I didn't consider that in my story I didn't ask for it. But you are right, they should be added in the 2.0 version of the survey.
Question 4: Besides the physical appearance I think the most important thing about the antagonist is creating a moral contrast between him and the protagonist to create the feelings of jealousy, rage, injustice. For example, making the antagonist as inmoral and the protagonist as moral as possible. When both of them are nice guys or both are scum there is less emotional impact.
Agree with you. But I also think that this should be made with some moderation. I hate when the netori guy is too much of an asshole or a sex-robot without emotions. I think a good antagonist have to have clear goals and motivations (as any good character should). Sadly I don't see that too often and the antagonist turns to be your random generic netori guy without personality. I've noticed most writers/developers/artists focus only in the MC and the heroine, neglecting totally the netori guy. However I've also noticed that the ones that are considered among the best (Rascou, Venus, Ver9, ANIM in the past... etc.) focus into creating a solid and sometimes even interesting, netori guy. I think that's a staple of the best writers in the genre.
