If it monopolizes the game I tend to zone out. For example if it's around an 6 hour common route and the actual romance/story telling is less than 5/6 hours I hate it. I enjoy the build up don't get me wrong but if it doesn't have variations or changes per route I shut it out and end up fast...
I'd enjoy more eroge Otome games but only if the protagonist actually looks like a woman and not a teenager. If the characters look like they should be in jail during the sex scenes I just stop playing. Size difference is fine but there's a line between that and questionable choices...
I enjoy more choices! As much as I enjoy an engaging story I also enjoy character building my own character when it has options too. It's more a western thing to have more choices but I do enjoy it especially when the protagonist is a self insert or a blank slate.

Definitely care more about the protagonist. She needs to have substance and something that grips me as her role in the story is much bigger than one of the LIS. Obviously the LIS need to be fleshed out as well but one or two bad ones doesn't put me off the game fully :)
The story and artwork mostly. It has to have an interesting premise for me to pick it up and even better if the MC has some sort of personality than just a doormat. VNs are always exciting when I notice the similar voice acting but otherwise I never really look it up.
My absolute favourite is the lovely Neji Kokuto. His route, story telling, character design, background is the best written broken character I've seen in a realistic setting. Also his voice is beautiful.

Honestly setting interest me first, the premise of the story. Where is it set, what age range are the characters. Then it's the artwork, I adore more unique art compared to the standard otome look so the more interesting the designs are the more likely I am to pick it up.
