Give me your best 18+ game recommendations (otome, specifically, because I've probably played all the BL ones already)

The title says it all. I am an avid BL gamer, although I play otome games as well if they suit my tastes. I'm looking for some spicy, 18+ recommendations. If it's a BL game, I've probably already played it... but I honestly can't think of many 18+ otome games, so please, pick out your favorites and sell them to me. I want something new and different.

The only one that comes to mind that I played recently was Heart Cage by RiceLoveCoffee - which I actually adored BECAUSE it was ridiculous. Between the yandere love interests, the love-craftian horrors, and the absolutely out-of-pocket dialogue, I am not ashamed to admit I had SO much fun playing it.

I also play a lot of BYOG (bring-your-own-gender) games, like A Date with Death and it's 18+ DLC, Bewitching Sinners and its 18+ DLC, and Error 143 and ITS 18+ DLC.

While I prefer games with localization or that were originally written in English, I have a decent set up with a translation app, so if the game sounds interesting enough, I'd still be willing to play it regardless or language or translation. I do prefer voice acting, but again, if the game is good enough I can live without it (Bewitching Sinners is always my go to example - no voices, but the art and the story are so good, I don't care).

That said, I have literally no kinks that I will shy away from, so feel free to throw anything at me. Give me 50 shades of love triangles, squares, or polygons, sprinkle in some yanderes, toxic behaviors, and other red flags. I'm fine reading about violence or sexual assault, as long as it has a place in the story. Wrap it in tentacles, and add a dash of incest, watersports, or necrophilia for flavor, I really don't care. As long as it's enjoyable and well-written, I'll give it a shot.

(And yes, I like good endings, but I LOVE bad endings.)

I want to recommend more titles like Princess Britannia and Trick or Alice etc, but I think the list will be too long lol. The Second Reproduction is getting a remake, so it's probably better to play the remake version next year!

Kusunoki Yashiki no Monogatari. Basically, a "Death Note" but, well, with 18+ instead of death. If you played or heard about Miracle No-ton, this is an otome version (kind of).

Also, you said you played most BL, but just in case there is a small BL 18+ game called Kazuma to Kugayama no Kimyou na Haikou Tanken. For a short story it is surptisingly well done in terms of art and plot.

The developer Mirai also made Sumire no Tsubomi, Hoshi no Oujo, Tsundere★S Otome -Sweet Sweet Sweet- & a couple other R18 otome.

I'll pose the same question to you as I did to @Daisy Magnus229 - are there any among the list that you would recommend over another based on the story? Because I'll absolutely check them all out, but if there are any you like better then the rest, I'd like to start there.

my personal faves are Chou no Doku & fandisk, Under the Moon & PersonA! I hope you like them :)

I'm probably bias lol both are good, but for me Kankinkon is the best (because I listened to the drama cd first and love the illustrator). This game is perfect if you like yandere characters, all Kankinkon LIs are yandere from severe to mild one. Dusk even have two more games, I'll include them in recommendation below.

If you're interested in Kankinkon, these two are highly recommended:

I've just remembered that the game itself has no actual sex scenes, screen just fades to black when 18+ happens 😅 But! The same creator have a CG pack with all the 18+ scenes missing from the game. The quality of drawing is 😳

If you're looking for a lighthearted and fun story and aren't too particular about the art style, give Fashioning Little Miss Lonesome a try.

If you're interested in adult themes like infidelity or illicit relationships but also want a story with depth, check out Aromarie's games (月ノ光 太陽ノ影/銀の冠 碧の涙/蝶の毒華の鎖).

If you're looking for well-crafted stories with emotional entanglements (psychological analysis?), try works by 松竹梅琉騎 handles the script (such as 越えざるは紅い花/幸の天秤, etc.).

If you're interested in a "moe" art style and erotic elements—and aren't too particular about the script, but prefer a cute, adorable art style—you might want to try Under the Moon and 赤ずきんと迷いの森.

There's actually a lot more to say, especially about the works of MIRAI, the company that developed the first commercial R18 otome game. However, those are quite old, so you can take your time getting familiar with them…

A lot of good ones have already been recommended on this list; I really like Torikago no Marriage (which was suggested here), though I should warn you that it's quite unconventional. I played Heart Cage a while back and loved it, too—though I do think the voice casting could have been better, but that's just my personal take.
My own recommendations are two titles from the same company!

I recommend Koezaru wa Akai Hana ~Itoshiki Hibi wa Mune ni Tsudoite~.
It has a variety of endings, and the story is interesting.

The Crimson Flower that Divides: Lunar Coupling《不可逾越的紅花~雙月~》is available on Steam.

The female protagonist, though not a true princess (adopted by the king), is intelligent, brave, and level-headed. She isn't blinded by love but instead constantly thinks about how to help everyone escape their current predicament.

She possesses excellent archery skills, doesn't rely on the male protagonist, and dares to face him head-on. She doesn't hide behind him for protection but fights alongside him.

The male protagonists are all different in age, personality, and type, making the game worth playing.

People have already recommended Princess Britannia, so I have to recommend Sanzen Sekai Yuugi ~MultiUniverse Myself~ by the same devs, girls★dynamics, they are a sister company to LoveDelivery (a bl dev) and all of them are under Holicworks.

If you are a BL connoisseur, you'll probably enjoy this one because it's the most BLesque otome (structurally/character wise etc) I played till this day. Like, it legit helped to get into otomes.

I'll bump Torikago no Marriage up on my list. I find unconventional is usually the most interesting. And thanks so much for the other 3 recs as well!

Regrading Heart Cage - I think there were one or two voices that threw me off in the beginning, but they grew on me after a while. I'm honestly just amazed that a fairly small and unknown VN was able to get full voice acting, and from some pretty well known names. I bought the game the minute I found it - there just aren't enough fully voiced English VNs out there, and definitely not of the 18+ variety!

@24ku & @藍色水珠 - Added to the list, since both of you are recommending it.

Regarding Sanzen Sekai Yuugi ~MultiUniverse Myself~, the art is gorgeous and the story looks right up my alley. Thank you so much! Anything associated with HolicWorks usually means quality to me, so I may end up playing this one first.
