For me as an outsider, my confusion is generally around random writings on body and that measurement ruler on the stomach (i mean i get what it means, i just don't get why it's hot)

The appeal of body writings or markings usually comes down to what they imply. Even though QOS has its own very specific racial focus, the core idea is pretty much the same: it's about a character visibly embracing a super sexualized or degraded role. The same goes for ruler markings or body writing like 'slut' or 'fuck me.' It's basically just a really in-your-face way of broadcasting that girls are slutty and very sexually active. It might be a bit of a clinical way to break it down, but that's essentially why people find it hot.
