I'm curious, how does one upgrade an existing game? Does the installer detect what you've already got and update accordingly, or do you need to do something special (ie uninstall and restart from scratch, only use certain individual updates, etc)?

The initial installer does nothing special but inserts some string values to your OS' Registry Hives. The patch installer will ask you the installation location if your OS' Registry Hives do not contain those strings, and the last resort is that you can copy&paste patch files manually (but you need to check the Update.lst file to make sure each patch file is the most updated one).
However, updates of CM3D (not CM3D2) do edit the exe file instead of just replace the old one. As a result, it might require a clean install from scratch and follow WIKI's patching order in order to make Mosaic remover, MODs and memory editor work.

In general, there are a few installers do stupidly search your entire hard disks which takes from a few minutes to even a few hours, and very few does optimised search which takes magically a few seconds. Most of installers rely on the Registry Hives.
