Some users have a horizontal line in their username and the word "Banned" written under their username. What does this mean?
Was the account deleted by the user themselves or deleted by AS? or is it something else?

It means those accounts were banned for breaking rules, but they'll still be there, whether the bans are temporary or permanent. The only one who can delete accounts are account owners themselves, to prevent account hijacking.
